Outpatient Appointment

Thank you for choosing to see me at West End Paediatrics, Harley Street, London, for your child’s outpatient appointment. This letter sets out some important information that I am required by law to provide to you. This is for your information only and is not a bill. As this includes information about my charges, if you do not have private medical insurance and someone else will be paying your bill, you may wish to pass a copy of this letter to them. Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charge which they do not pay.

Consultation Fee

All appointments, both initial and follow-up, are one-hour long as standard. My fee for an initial consultation will not exceed £450 (exc VAT) and my fee for any follow up consultation will not exceed £450 (exc VAT). This is correct as at the date of this letter. Payment must be made in full at booking. Should you cancel your appointment with more than 72 hours notice, a full refund will be applied. If you cancel with less than 72hrs notice or do not attend your appointment, your fee will be non-refundable. appointment fee may be charged.

You have an option to add on Extra time and other “add-on” services when you book your appointment. These may incure additional charges, and you will be invoiced for them at the time of your booking.

Following your consultation you may need certain tests (for example, blood tests or imaging such as an X-ray, MRI or CT scan) to help me diagnose your condition. If the test is undertaken by the clinic or hospital, and not by me, the fees for those tests will be determined by the clinic or hospital and charged to you, or your private medical insurer, separately.

If there are any fees which I will charge in relation to any of the tests I advise that you have, I will let you know what those will be.

My FEES ARE SELF PAY only but we can provide you invoices to claim from your Insurance company

Cancellation Policy

We require at least working days notice if you wish to reschedule or cancel your appointment without charge. For cancellations with less than 72 hours notice or if you do not attend your appointment, a cancellation fee of 100% will be charged.

Please note that most insurers will not cover a cancellation fee so you would be invoiced directly.

Private Medical Insurance

If you have private medical insurance, please contact your insurer before your consultation to check the terms of your policy, particularly the level and type of outpatient cover you have, including any reimbursement limits on individual consultation fees. I am recognised by most private medical insurance companies.

Please note that my fees will be charged directly to you, but we will provide you an invoice with which you can claim reimbursment from your Insurance provider.

Financial Interests

I am legally obliged to inform you that I, Dr Chinwe Ude, have a financial interest of 100% in Dr Ude Paediatric Practice Limited.

I look forward to welcoming you to my practice.

Yours sincerely,
